The Medical Genetics Institute at Cedar-Sinai Medical Center is conducting a survey to assess
the attitudes toward genetic testing within the Jewish community. As you may know, genetics is
an area of medicine that is rapidly changing and genetic testing, both before pregnancy and
during pregnancy, is becoming available for more and more diseases. 

We are currently conducting a questionnaire-based study, called “Attitudes toward Genetic
Testing in the Jewish Community” to learn more about people’s feelings towards genetic testing
and to see if there are any changes that should be made in how people are approached about
genetic testing. We are interested in learning the opinions of people of Ashkenazi background
and people of Persian Jewish background, to see if there are differences in how people of
different heritages feel about genetic testing.

We hope that you will be willing to assist in this research by filling out a simple questionnaire. 
The questionnaire should take no more than 10 minutes to complete. 

To protect privacy, this research is being done anonymously and there should be no risks to
participating.  The decision to participate or not participate will not have any effect on medical
care the participants or their family members receive at the Medical Genetics Institute or through
any other Cedars-Sinai Medical Center program. This project has been reviewed and approved
by the CSMC Institutional Review Board, which approves and monitors all human research at
Cedars-Sinai Medical Center to assure that the rights and welfare of research participants are
